FORMS FOR VOLUNTEERS

 Getting Started Info sheet  (131k)

Prospective volunteers should download this form to learn the process of becoming a TherapyPets volunteer. Includes schedule of upcoming Orientation meetings.

Veterinarian Information Form - Version 3 (40k)

This "Vet Form" is required by prospective volunteers who need this completed prior to attending their first Orientation meeting. Also this form is required for the annual renewal by TherapyPets volunteers. Please use only this latest version. **OPTION: Fill in on-line - Complete the first part before printing.** (1 page)

 2009-2010 TherapyPets Renewal Form (75k)

To be used by TherapyPets volunteers to renew their registration with TherapyPets. This one page document includes a Visit and Interest Survey, as well as Terms of participation by TherapyPets volunteers. **OPTION: Fill in on-line before printing.** (1 page)

 2009-2010 TherapyPets Sponsorship Form (110k)

To be used by TherapyPets volunteers to have their registration sponsored.. This one page form is to be used to sponsor a TherapyPets Team. There are three sponsorship categories: 1) In Memory of - 2) Family sponsorships - 3) Business/Corporate sponsorship. See form for details. Sponsors are for individual teams and last for each renewal period. Sponsors are recognized on the individual teams' badge as well as a sponsorship certificate. **OPTION: Fill in on-line before printing.** (2 pages)

I.D. Badge Photo Suggestions (24k)

Used by prospective new volunteers to provide a useful photograph for the official TherapyPets I.D. badge.

WAYS TO SUPPORT THERAPYPETS

TherapyPets Fashionware Order Form + Donations (60k)

Wear TherapyPets fashion accessories: Hats, t-shirts, sweatshirts (both regular and zipper/hooded), short-sleeved denim workshirts, black mock-turtleneck longsleeve pullovers, umbrellas, [and for our Volunteer teams only: bandanas, safety lanyards, patches and logo vests with zipper pocket.] Donations: Use this also to make donations to TherapyPets. As a recognized Federal 501(c)(3) non-profit organization, donations to TherapyPets may be tax deductible as the law allows. (1-page).

 Donate your car (or more) to TherapyPets  (113k)

Facilities should use this form to let TherapyPets know they want to have TherapyPets visits. Instructions included.

Facility Visit Protocol Form  (335k)

Use the Facility Visit Protocol Form for registering a facility with TherapyPets for official visits. Please note that duplicate copies are required: the original for TherapyPets, and a copy for the facility. Includes a cover letter for the facility administrator along with an instruction sheet. A Note on PDF forms:

(If downloading a file and such seems unfamiliar, it may be time to call in the eleven year old or other computer-savvy person of the family to help out at this point.)
